1. These terms & conditions apply to this Referral Voucher. By participating, voucher bearers agree to be bound by these conditions. Claims must comply with these conditions to be valid.
2. The promotion commences at 12:00am on 16 July 2021 and shall remain available unless Myphysio amends, withdraws, cancels or suspends the promotion at any time.
3. This voucher is only valid for Physiotherapy services.
4. Only ONE voucher can be used in one appointment. Referrer can refer as many new patients as possible, and is entitled to receiving an equal number of vouchers equivalent to the number of referrals. Multiple vouchers cannot be accumulated for use in one transaction.
5. Referred patient must redeem their voucher before this Referral Voucher becomes valid.
6. Referrer is entitled to a maximum of $25 off the total amount of a future Physio appointment of their choice (either Initial or Standard Physio Appointment only). Any unused value will not be credited into their account.
7. This voucher cannot be combined with any other offer or promotion unless otherwise stated.
8. Myphysio Health Clinics may, at its sole discretion and at any time, amend, withdraw, cancel, suspend all or any part of this promotion. Voucher bearer will not be entitled to any additional compensation in the event that the promotion has ended with no prior notice.

4. Patient will be required to show proof of identity and date of birth upon the day of appointment to redeem their voucher.
5. Voucher bearer is entitled to 50% off the total amount of one future Physio appointment within their birthday month (either an Initial or Standard Physio Appointment only). Any unused value will not be credited into their account.
6. This promotion can only be used once within the calendar year.
7. This voucher cannot be combined with any other offer or promotion unless otherwise stated.

Clinical Pilates 10+1 Classes

1. Payment

• Patients are required to pay upfront for a package of 10 Pilates classes. This payment will be credited to their account.
• Upon completion of the 10 paid classes, patients will receive the 11th class at no additional charge.

2. Class Attendance and Credit

• The fee for each class will be deducted from the patient’s account credit upon attendance.
• Patients must check in at reception before each class to ensure proper record-keeping.

3. Class Validity Period

• All classes in the package, including the free class, must be used within 12 weeks from the date of purchase.
• Unused classes will expire at the end of the 12-week period and are non-refundable.

4. Booking and Cancellation Policy

• It is recommended that classes be booked in advance to ensure a spot is available.
• Cancellations or rescheduling must be made at least 24 hours before the class. Failure to do so will result in the class being deducted from the account as if attended.

5. Refunds and Transfers

• The Pilates class package is non-refundable and non-transferable.
• Exceptions for refunds may be considered only for medical reasons, supported by a doctor’s certificate, and are at the discretion of the clinic.

6. Health and Safety

• New patients are required to undergo an initial assessment to determine their eligibility for the Pilates class.
• Patients must inform the instructor of any injuries, conditions, or changes in their health before participating in each session.
• The clinic reserves the right to deny participation if it is deemed unsafe for the patient to continue without appropriate medical clearance.

7. Liability Waiver

• By participating in Pilates classes, patients acknowledge that they do so at their own risk.
• The clinic and its staff will not be held liable for any injuries or adverse health effects resulting from participation in the classes.

8. Code of Conduct

• Patients are expected to arrive on time and wear appropriate attire for Pilates.
• Disruptive or inappropriate behaviour will not be tolerated and may result in termination of participation without a refund.

9. Changes to Terms and Conditions

• The clinic reserves the right to amend these terms and conditions at any time. Any changes will be communicated to patients in advance.

By purchasing a Pilates class package, patients agree to these terms and conditions.
