Why is thoracic mobility important for shoulders?

You have injured your shoulder and you find it hard to do your daily activities especially overhead tasks such as putting your clothes on and reaching for food from the top shelf of the pantry. You’ve tried different shoulder exercises you found on the internet but it doesn’t improve your symptoms.

Have you considered looking at your thoracic mobility (mid-back range of motion)? The shoulder joint is greatly affected by neighbouring structures. The shoulder blade is connected to the thoracic spine through many muscles. Postural dysfunction such as rounded shoulders and stiff thoracic spine can affect the position of the scapula and limit the ability for the shoulder to move throughout range of motion. A simply exercise can illustrate this relationship between thoracic and shoulder mobility. In a seated position, adopt a rounded shoulder and curve back then perform shoulder flexion (arms moving in front of the body above head), can you see that it is difficult to reach all the way to the top? Now, correct your posture, sit upright and perform shoulder flexion again, do you get a lot more range of motion? This hyperkyphotic postural dysfunction overtime can lead to poor biomechanics and muscle impairments.

What caused thoracic spine stiffness?

There are many causes for thoracic spine stiffness however the most likely reason is environmental causes. Prolonged periods of sitting hunching over a computer screen or overloading the shoulder from an activity can lead to this issue. Many tradesmen or overhead sports athletes (AFL, basketball, swimming) are susceptible or have experience this issue. Physiotherapy can help loosen those stiff joints and strengthen those muscles surrounding the thoracic spine to promote optimal shoulder function. Physiotherapists apply manual therapy techniques including spinal manipulation and joint mobilization to help relieve stiffness.
